All dancers, including adults, must register online to participate any of our programs or classes. Tuition is due on the first of every month and can be paid online or in the studio with cash or check. An annual registration fee of $35 is due with your first monthly payment (not required for Adult Open Division or summer programs). View tuition and links to online payment.

DISCLAIMER AND CONSENT

The undersigned parent and/or guardian of the student registered herein understands and acknowledges that there is risk involved in participating in the activity and training of dance and hereby agrees to hold Metropolitan Ballet, staff and /or assigns harmless of any damages, costs or other liabilities associated with any injury or illness of any kind in the event such should occur. Media Release * I give permission to Metropolitan Ballet to use the name, images, pictures and videos of me/ my dancer for any lawful purpose, including for example publicity, illustration, advertising, and Web content.

PAYMENT CONTRACT FOR STUDENT REGISTRATION - Not required for Adult Open Division

This payment plan requires advanced registration for the Metropolitan Ballet Academy for Fall, Winter and Spring. Tuition is to be paid in full or monthly installments regardless of the number of classes in any given time period as tuition is prorated. Payments under the monthly agreement are due on the first of each month. A non-refundable $35 annual registration fee is required upon enrolling. All payments must be paid to Metropolitan Ballet online or by cash or check. CANCELLATION In the event a student wishes to withdraw from the academy for any reason a 60 day written notice must be delivered by U.S. Mail or email to Metropolitan Ballet. Two months of forward tuition must be paid from the receipt of the notice and all past due tuition through that date must be paid to cancel this agreement. ACKNOWLEDGEMENT I hereby abide and agree to the Payment Contract with Metropolitan Ballet. My signature indicates that I have read and understand the above conditions and agree to abide with the terms of this contract.
